Water Street Health Services has expanded to better meet the needs of our community. In order to receive services, the following criteria must be met:
- Uninsured (no private or public health insurance)
- Below 200% of the Federal Poverty Guidelines (see income chart below)
- Lancaster County resident with Photo ID (Lancaster office)
Potential patients must register ahead of time to receive an appointment. Registration must be done in person by patient seeking services or care. Please do NOT call to register.
Please note that as of 1/24/2012, Water Street Health Services is not accepting any new dental registrants until further notice. Please return to this page in the near future for updates.
Potential patients may register on either Monday evenings from 5-6:30 pm or Thursday mornings from 8-9:30 am by bringing proper:
- Current photo identification
- Proof of residency
- Proof of income for ALL financially contributing household members
The first 10 qualifying registrants with all proofs and paperwork in good order will be accepted and processed on each of the registration days. Please note further that you will be asked to present this same information every six (6) months to re-certify and update your patient chart.
Acceptable Proofs of Identification
- Copy of valid PA driver's license or PA ID card (with current address)
- Copy of passport
- Copy of Alien Registration card
Acceptable Proofs of Residency
- Envelope of mail postmarked within last 3 months, with applicant's name and current address
- Copy of valid PA driver's license
- Copy of utility bills
- Copy of rent receipt or written statement from a non-relative landlord
Acceptable Proofs of Income
- Paystubs, checks, award letters from last 30 days
- Employer's written statement including employer's name, address, phone number and how much was earned during the last 30 days
- Business records and receipts, if self-employed
- Social security award letter
- Worker's Compensation check stub or current award notice
- Award statement for unemployment
- Pension check or statement
- Written statement from person or agency providing money or making payments for you
